There were a few versions of the form in the years of its use, but the general design and subjects covered stayed the same. The questionnaire was printed on 17” by 10” paper, folded in half to create a four page form.

In the facsimiles of questionnaires presented in this exhibit, please note that the markings and numbers, etc., in the margins of the forms were made by Joseph Dixon and show what information was of most interest to him.  Handwriting on the questionnaires can be difficult, so we include transcriptions in which we have corrected spelling errors but otherwise not changed the content.
